In December 2019, I had the opportunity to participate in BHGF setting up platform to study in the Siena University Hospital of Italy for three months.Supported by the leading of the department, and the care from BHGF and the staff of the University Hospital of Siena, I successfully completed a three-month period of communication and study. Although the final progress has speeded up to the ended as the result of global embolism of the novel coronal virus, nothing can ever frighten my marvelous impression away, not only the romantic customs of Italy but also the unforgettable teaching and medical of Siena University. several details of my overall program are shown as follows.
1. See something from the Siena University Hospital
The five-color triage system of emergency center is performed at the University Hospital of Siena.There are five colors comprising red,yellow,blue,green and white. Diverse degrees of severity of disease are tagged by these colors. For instance, the red and yellow marked patients need to treat urgently. Light patients need to wait for several hours to make way for the severe. Their philosophy is to give priority to urgency.
Moreover, when going through corridor to the inside of the hospital, the 6 color direction is shown. Different colors represent different buildings, which, at the same time, separately identify different department. with this frank direction, you can approach to your destination without any problem.
2. Department of the Neurology-related in Siena University
There are many neurological departments in the University Hospital of Siena.Now it is important to introduce Neurology Emergency，Pediatric Neuropsychology，Neurology intervention and Neuro-imaging. Neurology Emergency is the largest department for treating the disease in the Tuscany region, with 200,000 stroke patients per year. With close cooperation with rehabilitation department, all the physicians and neurology physicians function in an effective and creative way. All the 18 beds are used for treating stroke patient with and 8 of them are intensive care beds. Stroke unit of Siena University Hospital is authorized by Tuscany and the National Coordination Center that systemic thrombosis, within 3 hours after the onset of ischemic stroke, can reduce or completely reverse nerve damage. And following diseases or fields are mainly seen in the Pediatric Neuropsychology department: autism and general developmental disorders,Relt syndrome,Electrogram for children and adolescence with epilepsy, Communication and Speech disorders, Headaches for children and adolesence,Attention deficit hyperactivity disorder syndrome，Tourette syndrome，Developmental withdrawl and learning difficulties. Department of Neurology intervention and Neuroimaging is totally a diagnostic center,which in charge of intervention in the cerebrum, spinal cord, carotid and vertebral artery, as well as intracranial aneurysm，Dural fistula，acute cerebral ischemic and tumor. Recently, this department and the center of common eye cncer-retinoblastoma for treating children have cooperated to develop a new method named opportunity-selective ophthalmic artery chemotherapy by neurological intervention.
My tutor, Professor Dotti, who is very friendly and warm-hearted to us , is remarkably knowledgeable in medicine.She expertises in diagnosis and treatment of rare disease, such asHSP，GBS，MND，AD，CADASIL，CAA，PMD，CIDP，CMT，MS，Mitochondrial encephalomyopathy，Superficial siderosis of the central nervous system，Hepatolenticular degeneration，Friedreich ataxia，Polyneuropothy，Vasculatis，Muscular disease, and so on. Professor Dotti is very passionate about work, and her daily work schedule is absolutely full.As long as visiting a typical case, she will explain with us in detail no matter how busy she is. By the way, it has been a very helpful act that she also arrange a doctor who is good at English to communicate with us.
In addition to professional courses, Siena University Hospital also arranged a lot of public lessons for us, such as National and Regional health system，Health financing system，How to manage chronic outpatient diseases，Pain management，Hospital pharmacy and Gaelic laboratories, the Public Pharmacy visit，Gender differences in oncologyand so on. Thanks to all these lectures and classes, we got a broader view of Medicine and Italy.
with intense and busy work here, doctors in this hospital drink coffee occasionally and rarely have lunch. I am deeply impressed by the importance of human care for patient. The doctor fully communicate with the patient and patient also trust the doctor very well. When I knew the famous surgery robot Dafenqi and Rose technology was born in Siena University Hospital, I started realize that deep understanding of the mechanisms of each disease and carrying out research and discussions play a significant role in developing advanced technology. Hence, We should always keep a modestand alwaylearning mind and be involved in the multi-center study, which maybe the direction of our future efforts, like the notable saying of Steve jobs “Stay hungry, stay foolish”.
At the end of my passage, I want to thank my hospital and BHGF for giving me this precious opportunities to learn, which gave me more insight of western advanced medical technology in multiple levels and different customs in Italy. Although there are different social system and situation in different countries, I think Italian medical model is worth studying in many aspects, for example, emergency priority，organs donation，single disease，grading treatment, etc. Finally, I wish we have everlasting friendship and create a better future together.
